Welcome to Health Services. Authorization forms for administration of emergency injectable and oral medication are listed below.
There is not a school nurse at every school every day. Washington State law on oral medication administration allows non-mendical personnel to give medications at school. Staff giving medication are designated by the Principal and trained and supervised by the school nurse. On a field trip your child’s medication will be carried and given by their classroom teacher after receiving instruction by the school nurse. We request that medication be received in an original container with completed paperwork by a healthcare provider and also signed by a parent. All prescription and over the counter medications need orders signed by a health care provider. We ask that medications be brought to the school by a family member.
